President and Chief Executive Officer
Michael D. Slessor has served as the Chief Executive Officer of FormFactor, Inc. since December 2014, and as a Director since October 2013. Mike served as President from October 2013 to December 27, 2014, and as Senior Vice President and General Manager, MicroProbe Product Group from October 2012 to October 2013. Before joining FormFactor, he was President and Chief Executive Officer of MicroProbe from July 2008 through the October 2012 closing of FormFactor’s acquisition of MicroProbe. Prior to joining MicroProbe, he held various management, product-marketing, and applications-engineering positions in the semiconductor industry, primarily with KLA-Tencor. Mike received his Ph.D. in Aeronautics and Physics from the California Institute of Technology and his B.A.Sc. in Engineering Physics from the University of British Columbia.
CTO and Senior Vice President, Engineering & Product Development
Jarek (January) Kister has served as the CTO and Sr. VP Engineering & Product Development since January, 2014. Prior to this, Jarek served as the CTO & VP Engineering, MicroProbe Product Group from October 2012 to January, 2014. Before joining FormFactor, Jarek was the CTO and VP of Engineering of MicroProbe from April 2004 through the October 2012 closing of FormFactor’s acquisition of MicroProbe. Prior to joining MicroProbe, Jarek served as Vice President of Engineering with Kulicke & Soffa Industries, Test Division from 2000 to 2004. From 1990 to 2000, Mr. Kister served as Director of Engineering and later as Chief Technical Officer with Probe Technology, Inc. Jarek holds more than 30 U.S. patents pertaining to the semiconductor wafer and package test.
Mr. Kister holds an M.S. in Mechanical Engineering from the University of Michigan, and a B.S. in Mechanical Engineering from Polytechnic of Silesia, Poland. He is also a graduate of the Advanced Management Program from the Haas School of Business at the University of California, Berkeley.
Michael M. Ludwig
Chief Financial Officer
Michael M. Ludwig was named Chief Financial Officer in May 2011. He served as Vice President, Finance of FormFactor since December 2009, was a consultant to FormFactor from February 2009 to December 2009, and served as Vice President and Corporate Controller of the Company from April 2001 to April 2007. Mr. Ludwig has also held senior level finance and accounting positions at Force 10 Networks, Inc., a privately held company that builds and secures high performance networks, and at divisions of Tyco Electronics and Beckman Coulter. Mr. Ludwig holds a bachelor of science degree in accounting from California State Polytechnic University, Pomona.
Chief Information Officer
Mr. Eldridge was appointed Chief Information Officer in February 2014. Since 2001 he served the company as its Chief Technology Officer and as a Senior Vice President. After joining FormFactor in 1994, Ben led the development of MEMs MicroSpringTM technology for wafer-level testing of memory and system-on-chip (SoC) devices. He drove the creation and implementation of automation tools for design flow tracking, probe card and probe layout design, as well as the factory automation system used to manage FormFactor’s custom MEMs fab. Prior to joining FormFactor Ben was a research engineer at the IBM TJ Watson Research Laboratory in Yorktown Heights, New York for 10 years, where he worked on medical robotics and surface physics. He holds a Master’s Degree in Physics from Rensselaer Polytechnic Institute and a Bachelor’s Degree in Electrical Engineering from Union College. He is a named inventor on over 200 issued US patents related to MicroSpringTM technology and wafer test.
Senior Vice President, Marketing & Customer Solutions
Amy Leong has been with FormFactor since October 2012. Prior to this, Amy was the VP of Marketing at MicroProbe from April 2010 through the October 2012 closing of FormFactor’s acquisition of MicroProbe. Before joining MicroProbe, Ms. Leong worked at Gartner, Inc. as a Research Director from 2008 to 2010 and covered the ASSP system-on-chip and microcontroller markets. From 2003 to 2008, Ms. Leong worked at FormFactor where she served as Senior Director of Corporate Strategic Marketing and Director of DRAM Product Marketing. Prior to FormFactor, Ms. Leong worked in a variety of semiconductor process engineering and product marketing roles at KLA-Tencor and IBM.
Ms. Leong holds an M.S. in Material Science and Engineering from Stanford University and a B.S. in Chemical Engineering from the University of California, Berkeley.
Vice President and General Manager, Cascade Systems Business Unit
Dr. Claus Dietrich joined Cascade Microtech (now a FormFactor company) as part of the acquisition of SUSS MicroTec’s Test Division in January 2010. Dr. Dietrich was division manager of the SUSS MicroTec Test Division from 2005 until the acquisition in January, when he became general manager in Europe for Cascade Microtech’s probe systems and engineering probes. Claus began working at Karl Suss Dresden GmbH in 1991 as director of engineering, and become international product manager of SUSS MicroTec Test Systems in 1995. Dr. Dietrich holds a Dr.-Ing. in mechanical engineering from Technische Universitat Dresden.
Senior Vice President and General Manager, FormFactor Probes Business Unit
Matt Losey joined FormFactor in 2011 and has served as Sr. VP, Operations and Quality since January, 2015. Before this, he served as VP of Manufacturing since February, 2014. Prior to joining FormFactor, Matt has held various positions in the semiconductor and microsystems industries, including Applied Materials, Texas Instruments, Sandia National Labs, and notably Advantest America where he served as the VP of Engineering for the probecard unit acquired from Verigy.
Dr. Losey received his Ph.D. in Chemical Engineering from the Massachusetts Institute of Technology and his B.S. in Chemical Engineering from Stanford University.
Vice President and General Manager, Cascade Probes Business Unit
Mark has over 30 years of experience in a broad array of operations and engineering management positions in semiconductor fabrication. Prior to joining Cascade Microtech (in 2011), a FormFactor company, Mark spent five years as site director of ON Semiconductor’s high-volume 8” CMOS factory in Gresham, Oregon (purchased from LSI Logic in 2006). From 1998 to 2006 Mr. Stager was operations director of the same factory under LSI Logic. Mark joined LSI Logic in 1988 as a manufacturing engineering manager producing gate array devices. Mark began his career as an engineer for GE Intersil and Fairchild, where he was responsible for semiconductor package design and coordinating offshore assembly operations. Mr. Stager holds a BSME from California State University at Fresno, and an MBA from San Jose State University.
Vice President, Global Human Resources
Tim Mueller has served as Vice President of Human Resources since July 2016. Prior to joining FormFactor, Tim held various operations, engineering and human resource leadership positions within the semiconductor industry with Intel and Atmel corporations, as well as two-years leading an NGO community rebuilding effort in the wake of Hurricane Katrina. Tim holds a B.S. degree in Business Administration from San Jose State University, and a Senior Professional designation through the Society of Human Resource Management.
Senior Vice President, Systems Engineering
Uday Nayak has served as the Sr. VP Systems Engineering since January 2015. Prior to this, Uday served as the VP Systems Engineering, from September 2012 to January 2015, and as the Senior Director of Engineering from September 2009 to September 2012. Mr. Nayak has over 27 years of semiconductor capital equipment development experience including Electroglas where he served in various technical positions including Director of Engineering and earlier as VP of Engineering, Nikon Research Corporation of America and KLA Instruments. Uday holds 13 patents pertaining to precision mechanics, motion and control.
Senior Vice President, Worldwide Sales & Service
Robert Selley has served as Vice President of Worldwide Sales and Service since June 2016. Prior to its acquisition by FormFactor, Rob served as Vice President, Sales and Service for Cascade Microtech from September 2013 until June 2016. In this role he built a team and processes that significantly contributed to Cascade Microtech’s impressive revenue and profit growth.
Rob has over 20 years of global sales and business development experience, building relationships with the Global 500/Fortune 100 companies in both the semiconductor industry and in the network applications markets. Prior to Cascade Microtech, he led the international sales organizations at both Apcon and ONPATH Technologies. Rob played a key role in positioning ONPATH Technologies as a market leader prior to its acquisition by NetScout Systems in November of 2012. Prior to that, he spent 16 years at Electro Scientific Industries (ESI) in a variety of executive sales roles including director of both sales operations and Asia sales, and director of global sales for their Semiconductor Products Group. In addition, he served as general manager for ESI’s Laser Trim Solutions Group.
Rob received his BA in International Business from California State University Fullerton and his MBA from California State University San Marcos.
Vice President and General Counsel
Jason Cohen joined Formfactor as Vice President and General Counsel, in August, 2015. Jason has over 20 years of experience as an international corporate and technology lawyer. Prior to joining FormFactor, Jason held leadership positions at Ubiquiti Networks, EasyMed Technologies, STMicroelectronics, and Gemalto/Gemplus, managing a full range of legal affairs to support these public multi-national technology companies. From 1994 to 1999, Jason represented clients as a member of the Corporate & High Technology Group of Landels, Ripley & Diamond LLP, and in the litigation practice of Wilson, Ryan & Campilongo PLC.
Jason received his Juris Doctor from the University of California, Hastings College of Law, and a bachelor’s degree in economics from the University of California, Santa Cruz, with highest honors.
