Raising Concerns

As described in our Code of Business Conduct, below are means for asking questions and reporting concerns about ethics and compliance. An issue cannot be addressed unless it is brought to someone’s attention. All reports will be treated as equally important whether they are submitted anonymously or not.

If you see or suspect any violation of the law or our Code, or you have a question about what to do, talk to your supervisor and ask for help.

Sometimes, you may feel that it is not appropriate or effective to talk about an issue with your supervisor. If that’s the case, you have other options:

  • Email: [email protected]
  • Mail: FormFactor, Inc.
    Attn: Compliance Officer
    7005 Southfront Road
    Livermore, CA 94551
  • We also have a third party hosted anonymous helpline. This service is available to you 24 hours 7 days a week. All reports will be treated as equally important whether they are submitted anonymously or not. Remember to include sufficient information for a proper review and investigation. To file a report online, or via phone, use the following link: FormFactor Raising Concerns Helpline.

Contact Human Resources for issues concerning aspects of employment such as workplace conditions, disrespectful behaviors, compensation, recruitment, disciplinary actions or dismissals.

You are also encouraged to use these resources to report any concerns about actual or suspected violations or misconduct that are subject to whistleblower protections under any applicable law, including relating to fraud, gross waste under government contracts or subcontracts, or dangers to health, safety or the environment. Nothing in our Code, or in any of our policies or agreements, will be interpreted to limit or interfere with your rights to engage in activities protected by such laws.

We treat reports discretely to promote a safe and trustworthy environment for employees to come forward with concerns. We will treat the identity of persons submitting reports as confidential, subject to matters of safety and the need for an appropriate investigation and response.

Our Non-Retaliation Policy

We will not tolerate any retaliation against any person who in good faith asks questions, makes a report of actions that may be inconsistent with our Code, policies, laws or regulations, or who assists in any investigation of suspected wrongdoing.

Reporting “in good faith” generally means making a genuine attempt to provide honest, and accurate information, even if it later proves to be unconfirmed or mistaken.
